LLendForma

SBA preparation guide

Prepare the information before the official form.

An organized personal financial statement can make the official application easier, but it does not replace the current SBA form or the lender’s instructions.

Start with the current source

Form versions and program requirements can change. Confirm the current SBA Form 413 page and ask the lender which program, date, attachments, and signatures apply.

Organize the recurring building blocks

  • Cash, investments, retirement, real estate, vehicles, businesses, insurance value, and other assets.
  • Credit, taxes, mortgages, vehicle or student debt, notes, guarantees, and other obligations.
  • Income, recurring expenses, contingent liabilities, ownership, and co-borrower information.
  • Supporting schedules when one summary line cannot explain several properties or debts.

Use one “as of” date

Values and balances should describe one coherent financial picture. Identify older source documents and update amounts when they no longer support the statement date.

What LendForma does—and does not do

LendForma helps organize information and adjacent schedules for lender submission. It does not file an official government form, determine eligibility, estimate approval, or replace professional or lender guidance.
